What is a DVLA D4 medical?

If you’re applying for an HGV, LGV, PCV or C1 licence, the DVLA requires a medical examination before it will grant you the entitlement to drive. This is known as a Group 2 medical, and it’s recorded on a form called the D4. A doctor examines you, completes the D4, and the form is then submitted alongside your licence application.

The standards for a Group 2 medical are noticeably higher than the standard car licence medical, reflecting the fact that you’ll be responsible for a much heavier vehicle, often over long hours and long distances.

Whether you’re training for your first HGV licence or renewing an existing one, a DVLA medical is the one step you can’t skip — get it wrong and your application simply won’t be processed.

Who needs an HGV or LGV medical?

A D4 medical applies to anyone applying for, or holding, a Group 2 driving entitlement, including:

  • New HGV/LGV drivers applying for their first provisional entitlement (categories C1, C, C+E)
  • New PCV/bus drivers applying for categories D1 or D
  • Existing HGV, LGV or PCV licence holders due a renewal
  • Ambulance drivers, who typically need at least a C1 entitlement
  • Some taxi and private hire drivers, where the local council applies similar medical standards

What’s involved in the medical examination

A DVLA D4 medical is thorough, but it’s quick and straightforward. During the appointment, the doctor will typically cover:

  1. Medical history review — including conditions such as diabetes, epilepsy, heart disease, and sleep apnoea
  2. Vision assessment — checking that you meet the Group 2 eyesight standard, with or without glasses or contact lenses
  3. Blood pressure check — since high blood pressure is a factor in the DVLA’s fitness-to-drive assessment
  4. General fitness assessment — a broader check of your physical health relevant to driving a heavy vehicle
  5. Urinalysis — a simple urine test, often used to screen for diabetes
  6. Completion of the D4 form — signed and dated by the examining doctor for submission with your licence application

Bring the right documents: your driving licence, a form of photo ID, glasses or contact lens prescription if relevant, and a list of any current medications or existing health conditions.
