DVLA D4 Medical Examination

Required for HGV, PCV, bus, coach, lorry and taxi drivers renewing their vocational driving licence — conducted at our Dundee clinic and in Kirkcaldy in partnership with Direct HGV Driver Training.

The DVLA D4 medical form must be completed by a GMC-registered doctor and submitted as part of your application to renew a Group 2 (vocational) driving licence. Our GP conducts thorough, DVLA-compliant assessments and completes the D4 form on the day of your appointment. We serve drivers across Dundee, Tayside, Fife, and Angus, with a second clinic location in Kirkcaldy for the convenience of Fife-based drivers.

During your D4 appointment, the GP carries out a full physical examination covering eyesight (including colour vision), blood pressure and cardiovascular assessment, neurological function, and a review of any conditions that may affect your ability to drive safely. The completed D4 form is provided to you on the day to submit directly to the DVLA.

What to bring to your D4 appointment

Bring your current driving licence, the D4 form (if already provided by the DVLA), your glasses or contact lenses if you use them, and a list of any current medications. You do not need a referral from your NHS GP.

Fit-to-Fly Letters & Certificates

One of our most frequently requested services — a GP-signed letter confirming you are medically fit to travel by air, often required by airlines for passengers with health conditions, recent surgery, or late-stage pregnancy.

Airlines frequently require a fit-to-fly certificate from a doctor before they will permit a passenger to board. This may be required if you have a chronic health condition, have had recent surgery, are in late pregnancy, or are travelling with medical equipment. Our GP can assess your fitness to fly and issue a signed letter promptly — often on the same day as your appointment.

Issued same-day

Most fit-to-fly letters can be issued on the day of your consultation. Telephone consultations are often sufficient.

Video or face-to-face

Depending on your condition, a video or face-to-face appointment may be used. We advise on the most appropriate format when you book.

Airline-specific wording

We can tailor the letter to meet your specific airline’s requirements. Bring the airline’s form or template if they have one.

Medical letters and certificates from a private GP carry the same legal weight as those from an NHS GP. At OUR GP we issue all standard documentation — from a simple fit note to a detailed letter for insurance or legal purposes — following a brief consultation. Same-day turnaround is usually possible, and letters can be emailed directly to a third party on your request.

Medical documentation from a private GP is unfamiliar territory for many patients — questions about validity, turnaround, cost, and what’s needed are completely normal. If your question isn’t answered here, please get in touch before booking and we’ll be happy to advise.

Yes. Medical letters and certificates issued by a GMC-registered GP carry the same legal validity regardless of whether that GP works privately or for the NHS. All documentation from OUR GP is issued on clinic letterhead with the doctor’s full name and GMC registration number. It is accepted by employers, courts, insurers, universities, government agencies, and the DVLA.
In most cases, documentation is issued on the same day as your consultation. For fit notes and simple letters, a brief telephone or video consultation is often sufficient and turnaround can be very fast. More complex documents — such as detailed legal or insurance reports — may require a face-to-face appointment and a short drafting period, but we aim to provide these within 24–48 hours of the consultation.
No. You do not need to have seen us before. A brief consultation is required before any certificate or letter is issued — this allows the GP to take a short history, confirm the clinical basis for the documentation, and sign it appropriately. There is no registration requirement and no need to bring a referral from your NHS GP.
No — by default, we do not inform your NHS GP about consultations or documentation issued at OUR GP. If you would like us to send a copy of a letter or medical certificate to your NHS GP or another party, we can do so at your request. Your consultation and any documentation produced are private and confidential.
Please bring your current driving licence, your glasses or contact lenses if you use them for driving, a list of any current medications and their doses, and the D4 form if the DVLA has already sent one to you. If you have not yet received a D4 form, we can provide the standard version. The examination takes approximately 45 minutes and the completed form is given to you at the end to send to the DVLA.
In most cases, a fit-to-fly letter can be issued on the same day as your consultation. If you have an urgent requirement — for example, travelling within 24–48 hours — please contact us directly by phone or WhatsApp rather than booking online, and we will do our best to accommodate your timeline. A brief consultation (telephone or face-to-face depending on the nature of your condition) is required before the letter can be signed.
